To get started, fill out the form below. A member of our service team will contact you to schedule a 15-minute appointment with a Big Tree Weight Loss provider. To get the most out of your appointment, we recommend writing down your questions beforehand.
Please note: The cost of the appointment is $45.00, which will be collected when scheduling the appointment. If you choose to become a Big Tree Weight Loss member, this $45 will be credited toward your first month’s membership cost. You will have 7 days to decide if the membership is right for you. We require a commitment of 3 months for the weight loss membership program.
